HOW DO I...
OBTAIN A HOME VISIT?

Patients are expected to attend the surgery whenever possible, as we have better access to medical equipment and facilities here. If you need a visit please telephone before 10.00am. The receptionist will ask for full details of the patient and their condition. Details will be passed to the doctor for attention. Please note a visit is at the discreation of the doctor.

The surgery policy on home visits is that we expect children to be brought to the surgery where we can arrange for them to be seen as quickly as possible.

Please note lack of transport is not a valid reason for a home visit.

OBTAIN A APPOINTMENT?

We run a full appointment system. Appointments may be made by telephoning us on 0844 576 9140.

Routine appointments may be made two weeks in advance, enabling us to offer you an appointment at a time that may be more suitable to your requirements.

We also make appointments available on the day for urgent problems. These can be booked from 8.00 (morning) or 13.30 (afternoon), by telephone or by calling at the surgery.

If you cannot keep an appointment, please inform us as soon as possible as this will assist in over-subscribed situations.

HOW DO I...
REGISTER WITH THE SURGERY?

We can only accept new patients who live in our practice area; see the practice area map on the home page.

To register, you will need to:

• Complete a registration form and a questionnaire
• Provide proof of your address, for example bring along a utility bill, a bank statement or driving licence
• Bring along your passport if you do not hold a British passport
• Have an appointment for a health check with one of our practice nurses.

Please not that, if you move out of our area, you will need to register with a new surgery.

Our reception staff will be pleased to assist you if you have any registration queries, or require any clarification.
